SLAC Onsite location questionnaire

Required by SLAC that we know when and where you will be onsite.
Change that is required from 4/22/2021 is that this will now be for the week looking forward. The form will now need to be filled out by Friday 3PM for the following week. Form will then be reviewed and unless contacted you will be allowed on those days, and a report will be generated for planning purposes. If you find that you need to come in extra days, you will need to get (email preferred) approval from BM/ABM (Mark/Traci) as well as your supervisor in order to come onsite.
