This form is to be completed for EVERY session you sign up for to self certify you are fit to attend.  

If this is not completed by before the session, you will NOT be able to partake in the session

If you answer ‘YES' to any of the 'Fitness to Attend' questions you will be contacted by the Club COVID-19 Officer to confirm whether or not your attendance at the session will be sanctioned.
