I understand that these funds may not be distributed timely if I haven't submitted this form at LEAST 5 days prior to a Booster Club Meeting for review and vote.  *NOTE:  There are circumstances where at times the Board can meet outside of regular meeting dates for a Special Meeting, but not always.  Timing is key for our student athletes. *
Have you communicated with your team representative on the Board regarding your request?  We focus on communication and record keeping to help move the process along. *
I have emailed my Athletic Director and Principal regarding this request and included the Middletown Sports Boosters President (middletownsportsboosters@gmail.com), requesting approval prior to the MSBC's review of items requested.  *
I will attend or have a representative at the meeting where the vote will be cast by the MSBC, if there are any questions that arise. *
