** Purpose of this Contract

Members of the LWMB rent musical instruments and/or equipment from Lincoln-Way Community High School District #210 for the purposes of participating in this activity. The LWMB student (and by extension, the parent(s)/guardian(s)) is therefore responsible for the care and maintenance of the instrument and/or equipment throughout the LWMB season. The LWMB student and one parent must agree to the terms below in order to use the school owned instrument and/or equipment. Please read below and then complete the required blanks to certify that you understand the terms and conditions of this contract and agree to abide by them.

Students in the percussion, color guard, tenor saxophone (if necessary), bass clarinet (if necessary), trumpet (if necessary), mellophone, baritone, and tuba sections are required to complete this contract and pay the assessed maintenance fee in order to participate in the Lincoln-Way Marching Band.

** Terms of Agreement

The student is responsible for the care and security of this musical instrument/equipment. Should this item become damaged due to negligence, lost, or stolen, the student and the student’s parent(s)/guardian(s) will be held financially responsible for its repair and/or replacement. The replacement cost will be consistent with that of a new instrument or accessory of the same quality and will be assessed to the student and student’s parent(s)/guardian(s). All instruments/equipment MUST be checked in at the end of the LWMB season, or school year. The band director(s) / department chairperson(s) reserves the right to revoke the privilege of renting this item at any time.

** Maintenance Fee

A ‘Maintenance Fee’ will be assessed – $80.00 per extracurricular season or $150 for the entire school year. Fees are subject to change. Pay this fee in the LWMB Charms system.
