For interest or enrollment please fill out this form to the best of your ability. Please fill out a separate form for each student (after you complete the form it will present you with the option to submit a new one). Currently, The Conservatory is teaching both online and in person lessons. Our teachers are incredible at teaching through zoom and other video services, allowing the students to have an engaging, positive musical experience from home. Our great Newton facility is now available for in person lessons as well. We look forward to making great music with you!

Student's learning Style. We know that successful music education comes from pairing a teacher who's teaching style reflects the learning style of the student. This has been an incredibly effective way to learn music and everyone ends up having a blast in the process. Photo Release Waiver
The Conservatory PHOTO RELEASE FORM

I hereby grant The Conservatory permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of The Conservatory and will not be returned.

I hereby irrevocably authorize the The Conservatory to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge The Conservatory from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Cancellation Policy
For Private Lessons and Parent/Student lessons:

1. Every student is allowed one emergency cancellation without penalty per year. An emergency cancellation is any cancellation within the 7 days preceding the scheduled lesson.

2. You may cancel with 7 days advanced notice before a lesson with no penalty.

For Group Lessons:

No make ups or refunds are allowed for group lessons because the group meets at a regular time weekly like a class in school.

For Weekly Summer Sessions:

You can cancel your summer session up to a month before the start date for a full refund, 14 days before for a half refund. After that, you cannot cancel or get a refund. You can reschedule your summer camp session up to 14 days before the session.

For Lessons Canceled by the Teacher:

The student is not held accountable for the lesson price. You may have the option of a substitute teacher for the lesson if available.
