INFORMATION CLAUSE

As beneficiary of support offered in the framework of “reSTART” project, co-funded by The Asylum, Migration and Integration Fund and the government, I acknowledge that:
1. The administrator of my personal data is Foundation Ukraine (Leader – Implementer of the project), located at ul. Ruska 46a/201, 50-079 Wrocław, tel. 571 330 203 e-mail: iod@fundacjaukraina.eu
2. All matters referring personal data management may submitted via e-mail: iod@fundacjaukraina.eu, sent to the address provided in item 1 above or brought up via telephone number 571 330 203.
3. My personal data are/will be processed for proper realization of the project; especially for confirming eligibility of expenditure, providing support, monitoring, evaluating, controlling, auditing, and reporting, as well as for informational, promotional, statistical, and archival purposes and purposes of funds recovery.
4. The processing of my data is lawful and conforms to the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation).
5. The receivers of my data will be:
– the beneficiary
– the project leader, i.e. Foundation Ukraine, located at ul. Ruska 46a/201, 50-079 Wrocław
– the partners of the project: The Municipality of Wroclaw, located at pl. Nowy Targ 1-8, 50-141 Wrocław; Wroclaw Agglomeration Development Agency, located at pl. Solny 14, 50-062 Wrocław; The Kaleidoscope of Cultures Foundation, located at ul. Ruska 46a, 50-079 Wrocław; and MODE Foundation, located at ul. Energetyczna 14, 53-330 Wrocław
- European Projects Implementation Center of the Ministry of the Interior and Administration, located at ul. Puławska 99a, 02-595 Warszawa
- Ministry of the Interior and Administration, located at ul. Stefana Batorego 5, 02-591 Warszawa
along with subjects commissioned by the project leader and the partners to participate in the realization of the project. Personal data may be transferred for purposes of monitoring, reporting and auditing of the realized project only to subjects eligible to conduct the aforementioned procedures.
6. My personal data will be stored for 6 years from the approval of the final report by the delegated body (i.e. European Projects Implementation Center of the Ministry of the Interior and Administration).
7. The provision of personal data is voluntary, but a refusal to provide such data will result in being unable to receive offered in the framework of the project.
8. I have the right to access, rectify, object to the processing, and restrict the processing of my personal data. In this case the right to erase my personal data does not apply, as processing the data is necessary to fulfill the legal obligations requiring the processing of data under the law of the UE and of the administrator’s Member State.
9. I have the right to file a complaint to the President of the Personal Data Protection Office if I deem the processing of my personal data as violation of the GDPR.

CONSENT FOR PROCESSING OF SPECIFIC PERSONAL DATA AND PERSONAL IMAGE DATA

I hereby grant fully conscious and informed consent to the processing of my personal data with regard to my ethnic origin and to the processing of my personal image data by Foundation Ukraine, located at ul. Ruska 46a/201, 50-079 Wrocław, tel. 571 330 203 e-mail: iod@fundacjaukraina.eu. The data will be processed for realization purposes of “reSTART” project, co-funded by The Asylum, Migration and Integration Fund and the government, especially for confirming eligibility of expenditure, providing support, monitoring, evaluating, controlling, auditing, and reporting, as well as for informational, promotional, statistical, and archival purposes and purposes of funds recovery.

At the same time I declare that I was informed that in this case the right to erase my personal data does not apply, as processing the data is necessary to fulfill the legal obligations requiring the processing of data under the law of the UE and of the administrator’s Member State.

I also agree to receive marketing information from Foundation Ukraine and MODE Foundation, as well as information regarding events and trainings organized by the Foundations and a newsletter on other activities of the foundation via e-mail.
