ATHLETIC/VOLUNTEER PARTICIPATION WAIVER, CONSENT & RELEASE 

By completing this form, I hereby consent the individual named for participation in basketball sponsored by The Philadelphia Suns Inc. Furthermore, I hereby acknowledge that there are inherent risks associated and accompanied with the above stated sport/activity and that the individual named may be injured as a result of an accident arising out of participation in the named activity. In consideration for permitting the individual named to participate in basketball the undersigned releases and holds harmless The Philadelphia Suns Inc. and /or its employees, teachers, coaches, administrators, volunteers et al., from any and all liability including, but not limited to, liability for injuries or damages sustained by the individual.

PICTURE RELEASE

By completing this form, I hereby authorize usage of pictures and videos of the individual named for any lawful  promotional material of The Philadelphia Suns without payment or other consideration.
