Please read before completing:

There are rare occasions when the school is required to close the school site during the school day and/or send pupils home due to reports of a contagious disease.  These could include:

1. Positive Covid cases

2. Site closure due to hazardous conditions:
a) Snow / Ice
b) Heating failure
c) Flooding
d) Utility shortages or leaks (electricity, gas, water)
e) Hazardous materials on site


Please could you indicate on the form below whether you give permission for your child to be released immediately to make their way home (walk, public transport, own vehicle, cycle, scooter) without parents/carers collecting them.  This will enable us to reduce the number of pupils on site immediately in the event of one of the incidents outlined above.  If you have more than one child please could you complete as many forms as is necessary.

Please be assured, pupils will only be sent home without direct contact for the incidents outlined above. In the event of pupils needing to leave site for any other reasons (other than those indicated above) parents/carers will be contacted directly.
