I understand the commitment required for each team. I understand that I must sign a TEAM COMMITMENT CONTRACT once I have been selected for a team. I understand the time and financial obligations required as part of Mile High Dance Teams. I am prepared to be positive and accepting of my Team placement and I understand that any negativity regarding placements will result in my spot on any team being revoked. I understand that all communication should be in the form of writing and that I may request a private meeting to talk about my audition and results.

As the legal parent or guardian, I release and hold harmless Mile High Dance Center, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while in or upon the premises or any premises under the control and supervision of MHDC, LLC, its owners and operators or in route to or from any of said premises.
