Uniform Sizing Sheet/Order Form
Please use this form to submit your measurements to the Supply Officer to request a new uniform, a replacement, or an exchange.  
 
Fill in all measurements to ensure there is no delay in receiving uniform parts.

Once your uniform (part) has been sourced and ready for pickup, you will be contacted with an appointment time.

If this is a replacement/exchange request, what are you replacing/exchanging and why?  (ie.  parka - zipper is broken; glen - lost at camp).  Fill in the required measurement for the item you are replacing/exchanging.  
Head Measurement (required for beret or glen) - tape should be about a finger width above the eyebrow and level around the head
Neck Measurement (required for shirt) - leave a finger's space to ensure you are comfortable - you don't want it too tight!
Chest Measurement (required for shirt, tunic, t-shirt, parka) - measure around the fullest part of the chest
Waist Measurement (required for trousers) - measure the waistband of a comfortable pair of pants or around the waist of the body (avoid measuring on top of clothing if possible)
Hips Measurement (required for trousers) - measure the the fullest part of the hips and seat. Round up to the nearest 1/2 inch.
Height (required for shirt, tunic, parka, trousers) - measure against a wall or a door frame
Foot Measurement (Length, Width)
Hand Measurement (required for gloves) - You should use your dominant hand, the right if you are right-handed, and the left if you are left-handed.
