Dear M.S.Y.G. Parent(s)/Guardian(s):

THANK YOU for choosing to allow your child(ren) to join our Middle School Youth Group Community at St. Therese Parish!  M.S.Y.G. has been designed to meet the educational, spiritual, emotional, and social needs of our middle school youth (grades 6–8).  As part of the Catholic Church’s mission to evangelize and spread the Gospel message of Jesus Christ, we would like to provide a more in-depth and personal opportunity for young people to grow their faith.  M.S.Y.G. meets most Wednesday evenings from 7:00-8:30 PM in the St. Therese Parish Social Center.

To complete registration (1 thru 5):

     1.  Fill out and submit this online form.

     2.  Attend the FIRST M.S. Youth Group with your child on Wednesday, Sept. 11, 2019, starting at
          7:00 PM in the Social Center, doors will open at 6:30 PM.

     3.  At this 9/11 youth group, review and sign forms WE will print using your pre-filled responses,
          after which you may leave and return at 8:30 PM for pick-up.

     4.  At this 9/11 youth group, we request a family contribution of $150.00 per child.  To facilitate
          ease of processing:

                    *Please prepare your CHECK--payable to "St. Therese Parish" with a MEMO of your
                      child's FULL name
                                                                 --OR--
                    *Give online no later than 9/8/19 (FUND: "Middle School Youth Group" and include a
                      MEMO of your child's FULL name under "Special Intentions"):

     5.  At this 9/11 youth group, if this is your child's first year with M.S.Y.G., please bring and
          submit a copy of your child’s Baptism Certificate.  (For those who have not submitted one in
          the past, please provide us a copy.)

THANK YOU for taking the time to fill out this online form carefully and completely.  All responses will be treated with the utmost respect and confidentiality.  We would appreciate it very much if you could submit this online form as soon as possible (but no later than Sunday, Sept. 8th) to allow us time to print and prepare your official pre-filled forms for the First Youth Group on Sept. 11th at 7:00 PM.
