Wisconsin Dells 2019 Online Permission Slip
NOTICE:

You are signing up to be on a WAIT LIST for the Wisconsin Dells Trip. You will be contacted if other students drop from the trip in order to claim your spot. First come, first served.

CAMPUS LIFE WISCONSIN DELLS is July 8th to July 12th.

Students will not officially have a spot on the trip until their permission slip (either paper OR online), medical form, and $50 deposit have been made. (The deposit goes towards the cost of the trip.)

NOTES:
- The paper permission slip includes the same information as this online permission slip and the online medical form. Students can submit either a paper copy of the permission slip OR have a parent fill out both the permission slip & the medical form online.

- The Medical Form for the 2018-2019 school year is good until August 31st, 2019. If you have filled this form out anytime during the 2018-2019 school year, you need not fill it out again (unless your medical information has changed since you last completed the form).

CODE OF CONDUCT
Our desire is to provide the best possible atmosphere throughout the trip. Students are expected to cooperate with staff at all times and to participate in all scheduled activities. Possession and/or use of alcoholic beverages and/or any type of drugs/vaping is strictly prohibited. Failure to remain within these guidelines at any time is cause for the student's immediate to return home at the parent's/guardian's expense. We have read the code of conduct and agree to abide by it.
Parental Permission
I agree not to hold CAMPUS LIFE responsible for any injury that may result from my son's/daughter's participation in the trip to CAMPUS LIFE WISCONSIN DELLS  -  July 8th - July 12th, 2019. I also authorize CAMPUS LIFE STAFF to secure any emergency medical treatment necessary during the trip and I further assume all responsibility for the decisions so made.

I hereby grant permission to CAMPUS LIFE the right to use, reproduce, and/or distribute any photographs, film, video-tapes and sound recordings of my child.

PAYMENT
I understand I am responsible for making full payment for WISCONSIN DELLS by the required deadline. To reserve your spot, a $50 non-refundable deposit is required along with the previously mentioned forms.

PLEASE MAKE SURE YOU HAVE SUBMITTED YOUR REGISTRATION INFORMATION BEFORE GOING TO THE PAYMENT PAGE.

The trip price for students who sign up by the early deadline (May 31st) is $179.  The trip price after this date is $199.
