If you are applying for College Board accommodations, please have the student create an account at account.collegeboard.org before you complete the form.

Please note that all requests are completed online, and therefore CCHS must submit all documentation electronically.  CCHS has electronic copies of IEPs, 504s, and ISSPs, as well as any testing conducted by CCHS.  Any other supporting documentation needs to be provided by the family.  Please gather all supporting documentation that provides further insight in the student's disabilities and/or diagnoses (including doctor's notes and outside evaluations) and email or send hard copies to Alison Nowicki (anowicki@concordcarlisle.org).  Once we verify that all documentation has been received, we will proceed with completing the application.  Please note that it can take up to 8 weeks for determinations to be made.
