In consideration of the opportunity afforded me to use the services and facilities provided by Denver Wrestling Club in connection with my membership or contemplated membership in the Denver Wrestling Club, I freely and voluntarily make the following releases, statements and acknowledgments which I understand are legally binding upon me.

I declare and acknowledge that I am 18 years of age or older, and that I am in good health and capable of undergoing the physical exertion of wrestling and exercises for which I have voluntarily joined or contemplate joining. I have been advised by the club that if I have a medical history of any physical disorder or any doubts concerning the state of my health I should consult a physician before undertaking and exercise or wrestling for which I have joined the club.

I assume full responsibility and risk for any injury or damage which I might sustain directly or indirectly from any cause related to the wrestling program by the Club including, but not limited to, use of the various facilities offered by the Club. I hold harmless the owner(s) of the various homes, offices or facilities which the club has been allowed to use for its wrestling activities. I also acknowledge and agree that the Club or the owner(s) of the facilities being used are not responsible for personal belongings or holding or any loss or theft thereof, and I assume full responsibility and risk for any such loss or theft.

I hereby release and forever discharge Denver Wrestling Club and each and all of the owners of any facilities used by the Club, their owners, heirs, assigns, agents, directors, officers, partners, employees, representatives, attorneys and all persons acting, by, through, under or in concert with them of and from any and all manner of action or actions, causes or causes or action at law or in equity, suits, debts, liens, contracts, agreements, promises, liabilities, claims, rights, obligation, demands, damages, losses, cost or expense, of any nature whatsoever, known or unknown, fixed or contingent (collectively referred to herein as “claims”), which I now have or may hereafter have against each or any of the above persons or entities by reason of any matter, cause or things whatsoever from the beginning of time to the date of this document, and specifically including any claims which I may now have or hereafter have against each or any of the above persons or entities by reason of my use of the facilities used by me as a member or contemplated member of Denver Wrestling Club. This release is binding upon me and upon my successors, heirs and assigns, and I intend this to be a complete release and discharge of all persons as well as any corporate entity having anything to do with my wrestling activities offered by Denver Wrestling Club or the operation and maintenance of the Club.

This document is effective on and as of the date upon which I have signed it without regard to whether or not I joined the Denver Wrestling Club.

For the health and safety of our members, Denver Wrestling Club has adopted the following guidelines to both align and comply with current public health measures mandated by Denver Public Schools and the City and County of Denver.  The following guidelines are effective as of March 4, 2022.

1.  Masks and face coverings are optional for Denver Wrestling Club members while inside the wrestling room and on the mats participating in warm ups, training, and open mat time.  Denver Public Schools does strongly encourage the use of masks while inside school property, so we ask all Denver Wrestling Club members to wear a mask while inside other areas of the school such as restrooms, hallways, and while entering and exiting the building.

2.  All Denver Wrestling Club members must refrain from attending club training sessions if experiencing any symptoms of COVID-19 or other illnesses, such as colds and influenza.

3.  All Denver Wrestling Club members and coaches must be fully vaccinated against COVID-19.  Proof of vaccination required to become a member during the 2021-2022 wrestling season.

I HAVE CAREFULLY READ THIS ENTIRE DOCUMENT BEFORE I ELECTRONICALLY SIGNED THE ORIGINAL.I FULLY REALIZE THAT THIS IS A BINDING LEGAL DOCUMENT WHICH AFFECTS ANY CLAIMS I MIGHT HAVE HAD RESULTING FROM MY USE OF THE FACILITIES OFFERED BY DENVER WRESTLING CLUB BEFORE, DURING AND AFTER MY JOINING THE CLUB.
