To plan for the reopening of our schools in September, we need families to fill out this short form. PLEASE COMPLETE ONE FORM FOR EACH CHILD ATTENDING CHSLSJ IN SEPTEMBER. After submitting the form, there will be an option to complete another form for any additional children.

For September, CHSLSJ is planning for two instructional learning models, for all students in all grades.The first model, hybrid learning, where 2-3 days will be in-person and the other days will remote. The second model a 100% distance/online student learning environment. Please tell us your preference for your child. *
