I would like to send my child to school for as much face-to-face learning as possible.

I would like to send my child for a blended experience where they receive some face-to-face learning but learn and practice from home as well.

I would like for my child to learn virtually, as they did at the end of the 2019/2020 school year.

If school resumes in the fall with face-to-face instruction, which of the following best represents your thoughts on student transportation? *

I would let my child ride a bus to school with safety precautions taken, understanding that current levels of social distancing may be difficult to maintain.

I would let my child ride a bus to school but only if students were spaced 6 feet apart (every other seat).

My child normally rides a bus, but next fall I plan to bring them to school.

My child is not a bus rider.

I would like my child to start the year online and would not need transportation.

If school resumes in the fall with face-to-face instruction, which of the following best represents your thoughts on food service and the school cafeteria? *

I would feel comfortable with my child eating in the cafeteria with safety precautions taken, as feasible.

I would prefer that my child be provided a pre-prepared meal from the cafeteria to eat in the classroom.

I would send my child with a meal from home to eat in the classroom.

I would send my child with a meal from home to eat in the lunchroom.
