Thank you for pre-registering for West Angeles CDC and UCLA Pilipinos for Community Health (PCH)'s SOUL Community Health Fair!

During the event, attendees will have access to see health care providers and receive free medical screenings such as Pfizer COVID-19 vaccinations, A1c/glucose screenings, blood pressure screenings, vision exams, and much more. There will also be multiple health education workshops and and resource booths to promote holistic well-being and quality of life. These include housing resources, entry-level job opportunities, a Chair Yoga class, and a dentistry demonstration for children.

Dulan's Soul Food will provide FREE lunch to the first 100 attendees that visit a certain number of booths at the health fair.

When: October 23, 2021 from 11am-3pm
Where: West Angeles Cathedral (3600 Crenshaw Blvd, Los Angeles, CA 90016)

Disclaimer: For the safety of all attendees and volunteers, **masks will be required** for the duration of the event. If you are feeling sick or experiencing symptoms of COVID-19, you will not be able to enter the venue, but will be provided with resources for other health care providers.

Per Los Angeles County Department of Public Health (LACDPH) Guidelines, masks must be worn at all times for healthcare settings, such as the SOUL Community Health Fair. Masks will be provided for attendees who do not have one upon arrival. Even after submitting this pre-registration form, attendee's will go through a temperature and COVID-19 symptoms check before given access to enter the health fair. *
