Release Copy of California Immunization Record
San Diego Unified School District uses the San Diego Immunization Registry (SDIR), part of the California Immunization Registry (CAIR), for our immunization records.  Hospitals, clinics, and other health care providers enter the COVID-19 vaccines they administer into this database. The information in the registry is confidential and is not released to any unauthorized person or group.

Employees who are fully vaccinated, defined as being two weeks after the final dose of COVID-19 vaccine, will not have to complete asymptomatic COVID-19 testing.

By completing and signing below, you are requesting SDUSD’s Nursing & Wellness Program to access your immunization records and provide a copy of your immunizations to SDUSD’s Human Resource Department for the purpose of waiving the asymptomatic COVID-19 testing requirement.
