Thank you for registering your child(ren) for the Religious Education Program at Community Unitarian Universalist Church.

Please fill out the following information to help us better meet the needs of your child this year. (Note: A new form is needed each year, so that we can track the needs of children currently active in RE.) Membership is not required for registration.

A list of allergies will be made available to our RE classroom volunteers. In general, your information will be used ONLY in connection with the CUUC RE program and our Board of Trustees. Your information will not be made publicly available, sold or shared without express permission.

You can enroll as many as 5 children on this form. If you'll need to register more than 5 children, please fill out a separate form.

RE is a cooperative program that requires the gift of time and energy from many people. Because parent involvement is so vital to the success of our program, all families are encouraged to volunteer in whatever capacities they are able to participate. Please check your preference at the end of the Parental Information section.
