By completing the following questions, you give YOUR child permission to participate in the Cottage Lane Chorus program with Ms. Greene during the 2021-2022 school  year.  You understand that the classes will meet during the school day.  We will follow DOH and CDC guidelines in regards to singing indoors and outdoors.

Your child will have to miss class occasionally and is responsible for getting any work missed from their teacher.  
Classes will consist of mostly in person singing and possibly at home videos.   I will have a better idea of how often I can see them once I know how many children are interested in singing.

Please respond by October 4, 2021

Chorus will begin when the instrument lessons begin, usually around the end of September for 4th and 5th grade and in October for 3rd grade.
