Thank you for submitting our booking form for our Vanquish class on Wednesday 23rd September 2020 7.30-8.30pm at Sarisbury Green Community Centre. By completing the below form, you have provisionally booked your place for this week's class and will abide by the provisions required to keep Vanquish running and COVID Secure.

Measures include:
-Signing a Declaration Form before attending (You will not be able to partake without submission), if you haven't already done so; Please note, this data will be used for Test and Trace also
-Keeping 1m+ apart from other participants
-Bringing own water bottles
-Cash brought correctly and in an envelope with your name on it to prevent cross contamination. Online payments are available via our website shop: https://www.steppinup.co.uk/shop. We advise awaiting confirmation to purchase your class, if choosing online
 -Wearing comfortable, dance appropriate footwear at all times
-Awaiting outside the building until asked to enter
-Wearing a face mask when entering the building (Can be removed once in the dance space)
-Do not attend class if you are unwell with COVID symptoms, or have been in contact with another testing positive

Please note, you will receive confirmation shortly after completing this form, enclosed with our declaration form, if not already submitted. Our classes are of limit of 15 people, so please be aware, we may not be able to confirm your spot if maximum numbers are reached; We will let you know if this is the case as soon as possible and pop you on our waiting list if anyone drops out.

If you have paid for block blocking,  you will still need to complete this each week so we know which sessions you are attending, as this discount does not mean you have to attend 10 consecutive weeks.
