Math Course Approval Form
Use this form to apply for Math Department approval for transfer courses. 
-Use one submission for each course for which you would like to receive credit. 
-This form may also be used to apply for advanced placement for Cambridge A-levels and for IB-HL. 
-This form should not be used for courses taken during high school unless they were taken at a college. Credit is not offered for courses taken at a high school. 
Your submission is editable. At the end of the form you can opt to receive a link to your submitted responses. It will include an opportunity to edit your answers.

Please do not submit the form without the information requested. Doing so will result in delays.

You may need to contact the department at the college where you wish to take the course if this information is not readily available. The course can not be approved without sufficient information. (We except MCC courses from this requirement. Just enter "MCC".)
The information linked here also should show how grades are assessed. At least 50% of the grade should come from exams.

If you mean to take the course in the summer, please provide a link to a calendar showing the specific course is at least five weeks long. DO NOT just resubmit what you provided in the previous box unless it contains the appropriate information. Your course will not be approved without this information. (If this is not a summer course, enter NA.  We also except MCC courses from this requirement. Just enter "MCC".)

Have you taken this course at UR? If you have passed this course with a D- or better at UR, you are not able to repeat the course for credit at another college. You should consult with your advisor about next steps. *
