Grant Application Agreements:  1) I agree that I may be interviewed to further clarify or discuss the project/event/funding.  2) I agree to provide photos, videos, receipts and a report on the used grant funds.  3) I agree to return funds not used according to the grant agreement.  4) I accept this request may not be awarded, but that I can apply for other projects.  5) If awarded, I agree to sign a grant agreement prior to receiving grant funding.  6) If awarded, I agree to submit an invoice and W-9 for the award *
