Directory Information Form
Due to the pandemic, we have been delayed in putting out a new church directory. We are hoping to produce a printed church directory for this year to be distributed in June. To help us make this directory as accurate as possible, please fill out the form with the most current information. 

If we do not hear from you by May 1, 2023, we will assume your member information does not need to be updated. 

We will also be updating our pictorial directory for the year 2024.
