Please follow the steps below in order to complete the application process.

1. Please complete this online from in its entirety.

2. Please mail a check for $2,250 per team, payable to the NAPHL, 5151 Belt Line Road Ste 877, Dallas, TX 75254

3. Applicants that are accepted will be notified no later than June 1.

4. Applicants that are not accepted will be refunded any fees paid.
