2020 Winter Cup - Team Registration Form
Too cold outside and want to play soccer?

Come play at our Canada SCORES Winter Cup - an indoor co-ed charity soccer tournament!

When: Saturday, January 11, 2020, from 1:30 pm to 7 pm
Where: Futbol 5 - 109 Braid Building C, New Westminster (next to Braid SkyTrain Station)
Why: Help support Canada SCORES’ free after-school programming, focused on literacy and soccer, supporting vulnerable kids in Burnaby, New Westminster, and Surrey.

Tournament Details:
> 5 vs. 5 (including goalkeeper)
> Co-ed (1 player of the opposite gender on the field at all times)
> Teams may have up to 10 registered players on their roster (must be 19 years old and older)
> 1 division of play

If you are interested in playing, please complete the following team registration form for your team.

Within 2 business days, we will send you a Square Invoice via email for the team entry fee.
