A parent or guardian will need to sign off on the second and third pages of this form.  If they are not present right now, fill this form out at another time when they can sign the page with you.

Completing this form means that you agree with each of the policies laid out in this document.  If you are not able to agree to each of the policies listed on the form, do not complete this form.  You will need to complete this form to bring your device to school and use the WiFi at PA.
