We are so excited to have your child(ren) participate in Summer Counts! enrichment program. To best pair your child/ren up with the appropriate group and activities for the summer as well as provide you with program updates, we need some additional information from you! You will see several sections, this section will ask questions about you the parent or legal guardian, the following sections are questions about the child. You may skip to the end if you only have one child.
