Qualtrics + Mid-Term Survey Workshop Registration

This is a new workshop where you will learn about best practices for creating a mid-term course survey (and the benefits of having one) and then will create a Qualtrics survey and link it to a Canvas assignment.

Please use the form below to register for one of the session offerings.  Seats are "first come, first served" and will be limited so that we can provide hands-on support with setting up the Qualtrics survey.

NOTE: this workshop is currently offered only for OSU faculty and graduate students who are teaching Ecampus courses. It is not currently offered for non-OSU faculty or OSU faculty who have not taught at least one Ecampus course.
