To re-apply to become a Teen Educator in Oklahoma County please complete the following application. You will need to submit long form answers and three references. The ability to commit to all training dates and regular attendance at two evening meetings each month is required for all teen educators. Involvement in sports or multiple extracurricular activities may conflict with being able to commit to PATCH. Please let us know in your application any concerns or needs related to transportation and/or scheduling conflicts.

If you are under the age of 18, your parent/guardian’s permission will be required if selected as a Teen Educator.
