Hello 6th - 12th Grade Parents,  

This survey is meant to gather information from parents who are looking to use the Youth Center as an after school program primarily on the days your child attends school. (Youth who are doing 100% virtual learning are welcomed as well!)

By completing this survey you will be entering your child (or children) in the YC's lottery system. You are NOT guaranteed a spot. For more details on how this will work please go to: www.townofcortlandt.com/yc.

Forms must be completed by the deadlines listed below. If they are submitted after the deadline your child will not qualify for the lottery until the following month.

March Lottery due Sunday, February 14
April Lottery due Sunday, March 14
May Lottery due Sunday, April 11
