Screening Form for dyslexia

This is a screening tool, not a diagnostic test. The dyslexia Screening Test will indicate dyslexia and if there is a need for further assessment. If your student scores within the ranges that indicate dyslexia, it is advised that you seek professional help for the student.

Early assessment leads to early intervention. Any time dyslexia is suspected, screening should be done immediately and further evaluation.

Tick whichever is appropriate. If 70% of the answers are ‘Yes’, there is a need for further assessment.
