By submitting this form, you agree for Landmark to enter this information to an enrollment account where you agree to pay a prorate for the current month, prepay the first full month as well as a $12 annual maintenance fee. Auto billing will be scheduled to start the month thereafter. Payments will continue for the length of your contract. Additionally, contracts auto-renew monthly until a 14 day termination notice is given prior to the 1st of the following month.
