Children's Sunday School Registration Form

Sunday School begins on September 15 at 9:00 a.m. in the Fowler House! One Sock On, our fellowship group for parents of infants and toddlers, will resume at this same time in the Fowler House. Children age three through fifth grade can be registered using this form.

Please use one form per family. Multiple children can be registered for Sunday School using the short answer boxes in the first section of the form.
