白馬村内4スキー場どこでも滑れる1日リフト引換券が当たるキャンペーンへの宿泊施設申請フォームです。誘客事業等に関する連絡をしたいため、改めて登録していただきますよう、お願い致します。
※白馬村観光局会員施設はご登録不要です。
宿泊施設では、リフト引換券となる当選はがきに宿泊することを証明する意味で施設のサイン（又は施設印の押印）をしていただきます。

This is an application form for the accommodation to sign up for this campaign of which 1,000 people will be chosen by lottery to win “One-day lift ticket at any one of the Hakuba valley 4 ski resorts”.
At the accommodation facility, you need to sign the facility name (Or affixing a facility seal.) on the winning postcard to verify that the customer is staying at the accommodation.

当事業を実施する観光局に対し、本申請内容について情報提供することに同意します。 Agree to provide information of this application to the Tourism Commission that conducts public relations for the project.

暴力団員による不当な行為の防止等に関する法律（平成３年法律第77号）第2条第2号に規定する暴力団及びその構成員が本件申請に関わっていません。The crime group prescribed in Article 2, Item 2 of the Act on Prevention of Unjust Acts by Organized Crime Group Members (Act No. 77 of 1991) are not involved in this project.
