Insight School Small Group Permission Form

We are excited to begin our student support discussion group this semester. This group will meet virtually through class connect once a week. The groups will meet for 8 sessions starting Sept 29th.

Groups are optional and attendance is optional each week depending on the topic and how it relates to you.

Learning Coaches/ Legal Guardians:  If you would like for your student to participate in a Small Group, please complete this form.  

If you are 18 or older you can sign the permission slip yourself.

If you have any questions, concerns, or if you would like to discuss your student individually, please do not hesitate to contact your students counselor.
