Dear Farmington Families,

You and your student(s)are the reason public education exists. In a very short amount of time, FSD developed weekly lessons for students across the district. Each obstacle was viewed as a chance for growth. We need your honest feedback to better serve your students. The following survey is in sections that cover areas that were the district's focus areas over the weeks of district closure. The district will use this information to evaluate and refine our delivery and make necessary changes for AMI.

The multi-section survey will address the following:
