By completing this form, you are registering your son/daughter for 2020-2021 athletics.  You must still turn in a current physical to either the coach or the athletic office or have a current physical on file in the Athletic Office prior to tryouts.  A physical is considered current for 13 calendar months from the date it was obtained.
