How to vote by proxy:

1) Complete all 4 questions and click on submit.
2) Your proxy vote form  must be received no later than 6:45pm on Tuesday, December 13th.
3) You must be a member in good standing on the day of the AGM.
4) Your representative or alternate representative on your proxy form must be a member in good standing and be attendance at the AGM on December 13th.

Only ONE proxy per person attending is allowed.
