One of our primary concerns at St. Dominic School is the safety of our students. In keeping with this, every student who will walk home from school must have prior parental permission on file with the school office. This form will allow your child(ren) to walk home without adult supervision.

Please fill out a separate form for each child who will be walking home.

Please note, students in PK3-2nd grades will not be allowed to walk home without a parent-approved older sibling or adult.

When walking home from school, please remind your children of the following safety guidelines:
Go directly from school to your home. Students should only return to school in the event of an emergency.
Walk home using the sidewalk or walking path; cross only at designated crosswalks.
Obey all traffic signs and signals.
Check in with a parent or other designated adult/sibling when you arrive home.
