Registration form must be completed along with signed participation waiver, Code of Conduct form, Concussion Training Form and Image Release Form.

Please print documents and bring to 1st practice along with payment and, proof of physical dated within the last year. Your child may not participate unless all documents have been submitted. We appreciate your cooperation and understanding.
