If you plan to enroll your student in the 5th grade instrumental program, please fill out this information below. Transportation will then be able to prepare routes for 5th grade instrumental students over the summer.

The instrumental music program adheres to pandemic protocols as set by the Illinois State Board of Education.  As ISBE guidance is updated for 2021-2022, the format of this program may adjust in order to accommodate participating students within bus and classroom capacities.  Any changes in the number of lesson days, for example, will be communicated as early as possible.

If you are unsure if you qualify for busing, please call Unit 5 Transportation at 309-557-4287.
If you do not qualify for busing home from your JUNIOR HIGH school or you choose NOT to have your eligible students bused, it will be the parent / guardian responsibility to transport your student home on dismissal from the junior high at 3:45 pm.  ALL students will be bused to their lessons at the junior high school every day, regardless of eligibility. *
