GPHS Service Learning Hours Verification Form
Greetings,
Effective January 5, 2024 this form is no longer being used to receive Service Learning Hour Verification Forms. If you have already submitted electronically to this google form, your hours will be processed. Keep an eye out on Schoolmax over the next few weeks for updates. You will receive an email if there are errors.
All forms should be completed entirely and signed individually on each line for the date of service/volunteering. Please submit all forms in the guidance office in the proper location. Be sure to KEEP A COPY FOR YOUR RECORDS.
