Assumption of Risk and Waiver of Liability for Griffin Camp Authorization and Release 2020:                             The above-named child has my permission to attend the 2020 Griffin Basketball Camp (the “Griffin Camp”). I further acknowledge and agree that said child is subject to and is required to abide by the rules and regulations established for Griffin Camp participants. In case of injury or illness requiring emergency first-aid or medical treatment for said child, I hereby authorize and direct any adult Griffin Camp staff person or volunteer to intercede in my absence if such staff person or volunteer is unable to contact me or the designated person below in order for said child to receive medical treatment. In consideration of the above-named child being allowed to participate in the Griffin Camp, I hereby freely, voluntarily and without duress, acknowledge, agree and state as follows: 1. On behalf of myself and the above-named child, I hereby indemnify, release, hold harmless and forever discharge The Academy of Classical Christian Studies and/or First Baptist Church and its agents, employees, volunteers, officers, directors, affiliates, successors and assigns, including all Griffin Camp staff persons and volunteers, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I or said child ever had or may have, arising from or in any way related to said child’s participation in the Griffin Camp; provided, that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful or wanton misconduct. 2. This Authorization and Release is binding upon me, my spouse, the above-named child and our respective heirs, executors, legal representatives, successors and assigns. If any of the provisions of the Authorization of Release are held invalid or unenforceable by court of competent jurisdiction, the remaining provisions will continue to be fully effective. 3. This Authorization of Release is governed by the laws of the State of Oklahoma and is intended to be as broad and inclusive as is permitted by that law. If any provisions of the Authorization of Release are held invalid or unenforceable by a court of competent jurisdiction, the remaining provisions will continue to be fully effective. I have read and understand this Authorization of Release. I further understand that by signing this Authorization of Release, I have granted voluntary, complete and unconditional release of all liability on the part of The Academy of Classical Christian Studies and/or First Baptist Church, its agents, employee, volunteers, officers, directors, affiliates, successors and assigns, including all Griffin Camp staff persons and volunteers.                          Refund Policy: Refunds will only be given when we receive written notice from a physician stating that the athlete is physically unable to participate in the camp. Written notices may be emailed to rmckinzie@theacademyok.org. All approved refunds will incur a 20% administration fee which will be deducted from your refund amount. It is important to notify us before the first day of camp that the camper will be unable to attend. No refunds are given if a camper leaves during the camp week. *

Covid-19 Precautions:          • Participants are required to have shown no signs or symptoms of Covid-19 in the past 14 days.            • As part of the daily sign-in process, each student will undergo a routine contact-less temperature check and verbally give a health check-in.           • All doorknobs, railings, bathroom faucets, and related common-touch surfaces will be disinfected prior to each day’s camp.             • Students will make use of hand sanitizer stations before and after clinics, as well as during breaks.             • Gloves will be worn by concessions workers, and masks during food preparation.             • Balls and equipment will be sanitized between sessions and at the beginning/end of each day.                • During camp, coaching staff will enforce appropriate spacing in team and skill lines.               • Water fountains will be closed and all students will use personal water bottles or bottled water purchased from the concessions stand.                 • Spectators are limited to 1 parent per family, and they are required to sit in the stands/fan area with 6-foot spacing.                                                                                Covid-19 Precautions Acceptance Statement:    I/We agree to abide by these Covid-19 Precautions. I/We agree that these precautions are reasonable and acknowledge that these precautions comply with the applicable regulations from governing authorities in the state of Oklahoma. I/We acknowledge that there is an inherent risk of contracting Covid-19 at Griffin Camp and acknowledge that these precautions cannot guarantee a child’s safety from Covid-19 or a related illness. *
