Holy Family Sign-Up Form
Please fill out this information so that you can be placed in a group for Holy Family this year. This info will help us get in you in the right small group for your family. We will do our best to accommodate your requests. I know this has been a program for 2nd grader parents. My hope is to also this year have a few groups of Confirmation Parents if possible/there is a desire. So please fill this out if any of your children are going to receive a Sacrament this year.

Would you be a Lead Family (organize your group of 5 other families and help make sure dates get set, everyone knows about the meetings, locations are set, and will lead the discussion) I know that everyone could do this as the questions and discussion topics are in the book. It is a very easy job.  *
