This form must be completed by parents for any student (1) confirmed or presumed positive with COVID-19, (2) suspected of having COVID-19, or (3) exposed to confirmed or presumed positive COVID-19 prior to the student returning to any GISD campus. Upon submission, this form will be sent to the campus nurse and attendance clerk. Requirements for a student’s return to school can be found on the Student Exposure Process Map.
