Kindergarten Pre-Registration 2024/2025 School Year
Please submit this form if your child is a current preschool student with Springfield Local Schools. 

Once the form has been submitted you will be required to log into your existing PowerSchool account to update (re-submit) forms for your student. The last form will include a link to schedule your appointment with our Enrollment Coordinator, Kristin Brooks, to supply updated documents. Below you will find a list of the documents that will be required. Your child is not fully enrolled into Kindergarten until the meeting has taken place and all documents are received. Documents Required for Enrollment Meeting:
  • Current Lease (if applicable)
  • 2 Utility Bills dated within past 30 days
  • Parent/Guardian Drivers License
  • Immunization Records - showing 5 year immunizations
