In consideration of being allowed to volunteer at The United Way Lee, Hendry, Glades, and Okeechobee I hereby acknowledge that there are certain risks of injury involved, and I knowingly and freely assume all such risks and assume full responsibility for my participation. To the extent allowed by law, I agree to indemnify and hold harmless The United Way Lee, Hendry, Glades, and Okeechobee, its officers, employees, agents, representatives and volunteers, of all liabilities and all loss or damage to person or property which may occur or be incident to my involvement or participation.   Photo Release I give my consent to The United Way Lee, Hendry, Glades, and Okeechobee to use my or my child's image (photo or video) in any advertisement (social media, traditional media, promotion items, etc.) deemed appropriate. I also understand that The United Way Lee, Hendry, Glades, and Okeechobee will use the images exclusively for agency and mission-related purposes and not for any commercial gain. I agree that The United Way Lee, Hendry, Glades, and Okeechobee is not responsible for unauthorized use of the images, that I am not entitled to any compensation, and that the images may appear with or without my/our name.
