In submitting this form, I agree to donate the above-selected sponsorship in support of the above selected purpose of 100+ Women Who Care Montreal and/or door prize to the membership for the stipulated term(s).

In return, 100+WWC Montreal will make an announcement about our corporate sponsorship (1) at the corresponding event(s) (2) on their website along with our logo and our web hyperlink in the respective sponsorship tier and (3) in an email communications to the full membership (4) on their IG and FB social media accounts, as per outlined for each sponsorship tier in the 100+WWC Mtl Sponsorship Packages Table.

Gold or Platinum Sponsor: My payment will be made by credit card to the "100+ Women Who Care Montreal" Giving Group webpage on the Charitable Impact website. I will receive a charitable tax receipt from Charitable Impact.

Venue Sponsor: My payment is made three times a year to the Atwater Club, prior to our event, and a receipt from the venue will be provided.

I understand that if providing a Door Prize, it must be provided in time for the events.
