4th Annual Vista Hockey Alumni Game Registration
Are you ready to play in our annual Vista hockey alumni game?  Reserve the date, November 25th from 6-8pm at South Suburban Sports Complex.  Players should arrive by 5:00 to check in, and check out the silent auction items etc.

**Registration form must be completed by November 9th to be included in the program and to obtain the $40 participation and jersey fee***

LATE REGISTRATION FEE - If submitted after November 9, the cost to participate will be $60.  Please register early.  This event has become popular and we have limited spots so please sign up ASAP.  

Please register and provide the following details.  You will be notified by email no later than November 11th with a link to submit the participation/jersey fee.  You will need to pay the fee within 48 hours or receipt of confirmation to secure your spot.
